1. Service Agreement
All healthcare services provided by Probitas Healthcare Services are subject to a formal agreement outlining the type, frequency, and scope of care. Services begin only after mutual consent is established.
2. Client Eligibility
Services are available to individuals whose health assessments align with our care criteria. We reserve the right to accept or decline clients based on clinical appropriateness and service capacity.
3. Care Delivery
Care is delivered by qualified professionals in accordance with agreed care plans. Any changes in a client’s condition must be reported promptly to ensure continuity and safety.
4. Confidentiality
We are committed to maintaining the confidentiality of all client information in accordance with the Data Protection Act 2018 and GDPR. Personal and medical data will only be used for service delivery purposes.
5. Fees & Payment
Clients or their representatives agree to pay all charges associated with the services provided. Payment terms will be outlined in the individual care agreement. Late payments may result in service disruption.
6. Cancellations & Notice
A minimum of 24 hours’ notice is required for cancellation or rescheduling of services. Failure to do so may result in a cancellation fee unless due to emergency circumstances.
7. Liability
Probitas Healthcare Services will not be held responsible for any indirect or incidental damages resulting from the use or inability to use our services, except where required by law.
8. Complaints & Feedback
We encourage feedback to continuously improve our services. Concerns or complaints can be raised directly with our office and will be handled in line with our complaints policy.
9. Changes to Terms
Probitas Healthcare Services reserves the right to amend these terms at any time. Updated terms will be made available through official communication channels or upon request.
